Terms and Conditions

Agreement
I, (the customer) agree to pay BioLifeprint, a division of Foster Health (herein ‘the company’) the full cost of selected services. I affirm that, by selecting the Hair Tissue Mineral Analysis (herein ‘the analysis’) and 3 Month Custom Supplement Package (herein ‘the product’), I will pay the company the full cost as determined by the company, based on the results of my analysis and Expert Evaluation, on a monthly or quarterly basis, depending on my selection. I also affirm that I understand and subscribe to the company’s autoship policy, which ensures that I will receive a subsequent product, shipped approximately 2 weeks before the expected expiration of my initial product, and billed in the same way, and that I will continue to receive the product and be billed in this way, until I express otherwise to the company. I understand that the analysis and the product are non-refundable, and indicate my willingness to receive both.

Liability
I hereby waive the company and its affiliated laboratories, compounders, and practitioners of any liability associated with my analysis, recommendations, and product. I take full responsibility for any complications that may occur through the analysis, evaluation, and recommendations, and affirm that I am fully responsible for my use of the information provided by the said analysis, evaluation, and recommendations. I also take full responsibility for any complications that may arise from my use of the product. I understand that the company makes no claims to treat, heal, or reverse disease through its products, and I take full responsibility for any statements that I make to that effect.

Cancellation
In the event that I decide to cancel my product, I am aware that it is my responsibility to inform the company of my intention at least 2 weeks before my automatic shipment of the product. In the event that I cancel after that point, I understand that I will be responsible for paying for my next supply of the product, which I will be shipped according to the schedule. If I choose to cancel at any time, I affirm that it is my responsibility to pay the company the remainder of the cost of my current product.
